Surgery Consent Form

If your pet is scheduled for surgery, we require the Surgery Consent Form to be completed prior to the procedure. This helps us understand your wishes and provide the best care possible during your pet’s surgery process.

Dear Client,


Your pet is scheduled for an upcoming surgery. To help make the surgery day as smooth and stress-free as possible for everyone, we have prepared some simple forms for you to review and sign in advance. If you have any questions, feel free to call or email the hospital at (918) 653-2190 / hacdvm99@gmail.com. On the day of your pet’s surgery, you will be required to review and sign an Authorization/Estimate Form. We will also confirm that we have a phone number where you can be reached during the procedure.


The night before your pet’s surgery, please do not provide any food or treats after midnight. Water is fine and may be left out for your pet to drink. If your pet is currently receiving any medications, vitamins, or injections, please withhold the morning doses unless otherwise instructed by the doctor.


At the time of your pet’s scheduled surgery, the doctor will address any remaining questions or concerns and will collect the completed Authorization Form.


If you have opted for any recommended blood tests, they will be collected and performed before the start of surgery, unless they have already been completed in advance. If any concerns arise, the doctor may contact you using the number provided on the Authorization Form.


Once the surgery is complete, the doctor will process your payment, review all discharge instructions with you verbally, and provide a written copy. If you have any questions or need clarification on any instructions, please don’t hesitate to ask before or after your departure.
